Air accident and serious incidents investigations are done in accordance with the Civil Aviation Act, 2006, Civil Aviation Regulation, 2016 and International Civil Aviation Organisation (ICAO), Annex 13, in the interest of safety and with the aim of preventing similar occurrences in the future.

Air accident safety investigation is not for the purpose of apportioning blame or liability, rather, it is to uncover the cause(s) of occurrence and to propose safety recommendations, if applicable.


1. All accidents and serious incidents are required to be notified to the Accident Investigation Bureau (the Bureau);

2. The Legal responsibility for notification of an accidents and serious incidents rests first with the pilot-in-command of the aircraft involved, if he or she is fatally injured or incapacitated, then, the operator;

3. If the accidents and serious incidents occurs on or adjacent to an aerodrome, then, the aerodrome authority is also required to notify the Bureau;

4. The notification is required to be passed to the Bureau immediately, giving as far as possible, the following information: If it is an accident or serious incident-

(a) The type, model, nationality and registration marks of the aircraft;

(b) The name of the owner, operator and hirer, if any, of the aircraft;

(c) The qualification of the pilot-in-command of the aircraft and nationality of the crew and passengers;

(d) The date and time (UTC, Nigeria Time Zone) of the accident;

(e) The last point of departure and the point of intended landing of the aircraft;

(f) The position of the accident in relation to some easily defined geographical location;


(g) The number of the following:  

- Crew on Board (COB) and number fatally or seriously injured.

- Passenger on Board (POB) and number fatally or seriously injured.
